I have never heard of a cam chain coming off due to bump starting. The only difference is where the energy is coming from when cranking the engine.

I used to contribute to a forum for these cars and we had quite a few horror stories due to bump starting and tow starting also anything that caused the engine to turn in reverse.

I have read those threads and get the impression that the timing chain on that particular Ford engine aught to be replaced at the 100K service interval. Perhaps 75K to be on the safe side.
I also get the impression that the timing chain tensioner runs on the overhead pressure from the pressure regulator. This will explain why they often rattle at tick-over when the demand from the remainder of the engine takes priority at low revs.
