I was asked for the files of this project so I thought I would make a few "improvements" first.
In the first version the switching of the test card and test tone was done by switches on the phono sockets. This I found inconvenient as if the test card was wanted on its own a phono plug needed to be in the audio socket to turn off the tone. So in this new version I have used toggle switches to select the test card and tone. There is now a audio gain control on the front panel.
I have also changed the test card to the PM5544 style one.

Oh and I named it Vol as in Vol(e)
