It may well be that there's an occasional need to work on adjacent channels, as in the B4/B5 case you cited. Rather than develop a VSB modulator it just strikes me as easier to have a separate bit of co-ax going to the odd awkward set.
Darius developed a VSB filter for his converter many years ago. I've attached the schematic though it's not easy to replicate nor is there a plot of the filter response.
