What is the voltage prior to the regulator? Both off-load and on-load. For it to drop like a stone so suddenly suggests a number of possibilities.
An inadequate heat sink would show up after a period of operation. Depending on how inadequate, it might take several minutes before it plays up. So it's likely that the problem lies elsewhere. But having said that, it's still possible the heat sink will need upgrading - we'll see later.
