Doesn't look too bad.
Be careful tightening down the bearings. Plain bearings need a but of running clearance, and particularly if they're worn you can crack the casting if you give it too much welly.

Errrr. I don't think there is one. It's just a T-nut, and bolt, or bit of threaded rod and a nut if that's what's handy.
