There are many electrical therapy machines. From simple magneto-electric devices, cranked by hand to the well known violet ray machines which are popular toys in some circles. Many of these use high voltage low current and are generally safe in principle. This is subject to proper inspection and repair, the most likely danger is a breakdown between the mains input and high voltage sides which can put a human in contact with mains. There are also devices using lower voltage and higher current such as TENS machines. These are also safe if used correctly. Basically you must avoid passing current via the heart.
The ECT machine shown by the OP is most definitely not safe except when used by qualified practitioners. Even then I would have my doubts, ECT has largely fallen out of use as a therapy, at least in part for reasons of safety. The current levels are higher than those which can cause serious injury or even death if not correctly applied.
