Lever one end at a time, with cribbing under the other?

Yes Alan, that's how I did it, having built the pillars I left them for the cement to go off, the lintel was barred in front and centered, I used 4" concrete  blocks laid on the flat, one lift at a time, I used two other piles of blocks to lever from at either end, when at the right height the mortar was laid, one end was then barred over first then the other, to get it level I wedged the low end and packed more mortar in, when it had all set I pulled the wedge out...it was a bit of a balancing act...
