I'd get out the ohmmeter and check for continuity. I suspect you may be reading 'floating' voltages with no true reference to ground. It is just possible - but highly unlikely - that the internal connections of the bridge could have a fault, assuming that an encapsulated bridge is used. That can't be the case with discrete diodes, though. Resistance checks, that's my suggestion. Otherwise, put a dummy load on the bridge output and see if that brings the voltages down. As already mentioned by Refugee, wall warts are famously poor in regulation.
